The effects of adipokines ISM1,WISP1 on glycemic control and pregnancy outcomes in patients with gestational diabetes mellitus

Abstract
Objective:To investigate the effects of adipokines isthmin 1(ISM1)and Wnt1-inducible-signaling pathway protein 1(WISP1)on glycemic control and pregnancy outcomes in patients with gestational diabetes mellitus(GDM). Methods:A total of 180 GDM patients treated in Department of Gynaecology and Obstetrics of Sichuan Jinxin Xinan Women and Children's Hospital from December 2021 to December 2023 were selected as the subjects of this study.Based on the level of glycemic control during pregnancy,the patients were divided into a controlled group(100 cases)and an uncontrolled group(80 cases).Additionally,according to different pregnancy outcomes,they were categorized into a good pregnancy outcome group(108 cases)and a poor pregnancy outcome group(72 cases).The plasma levels of ISM1 and WISP1 as well as the homeostatic model assessment for insulin resistance(HOMA-IR)index were compared between the controlled and uncontrolled groups as well as between the good and poor pregnancy outcome groups.Furthermore,the univariate and multivariate logistic regression analyses were conducted to investigate the factors of poor pregnancy outcomes. Results:The plasma ISM1 level[(1.30±0.25)ng/ml vs.(0.78±0.18)ng/ml]in the controlled group was significantly higher than that in the uncontrolled group(P<0.05),while the WISP1 level[(422.43±70.19)ng/ml vs.(589.18±81.05)ng/ml]and HOMA-IR[(2.75±0.65)vs.(3.89±0.70)]in the controlled group were significantly lower than those in the uncontrolled group(P<0.05).The plasma ISM1 level[(1.50±0.34)ng/ml vs.(0.42±0.27)ng/ml]in patients with good pregnancy outcomes was significantly higher than that in poor pregnancy outcome group(P<0.05),whereas the WISP1 level[(448.67±78.80)ng/ml vs.(568.35±94.39)ng/ml]and HOMA-IR[(2.93±0.51)vs.(3.75±0.66)]were significantly lower in the good outcome group when compared with poor pregnancy outcome group(P<0.05).Multivariate regression analysis results showed that a family history of diabetes,a history of adverse obstetric events,high plasma WISP1 level,and high HOMA-IR were risk factors of poor pregnancy outcomes in GDM patients(P<0.05).Conversely,high plasma ISM1 level was a protective factor of poor pregnancy outcomes(P<0.05). Conclusions:The levels of adipokines ISM1 and WISP1 are associated with glycemic control and pregnancy outcomes in patients with GDM.Monitoring the levels of ISM1 and WISP1 in GDM patients can provide a reference for evaluating the effectiveness of glycemic control.Furthermore,it may help in the early identification of patients at risk of adverse pregnancy outcomes.关键词
