Abstract

Objective:To investigate the value of ultrasound blood flow parameters combined with the levels of maternal serum pregnancy-associated plasma protein A(PAPP-A)and placental growth factor(PLGF)for evaluating fetal growth restriction(FGR). Methods:Pregnant women diagnosed as FGR in Northwest Women's and Children's Hospital from January 2023 to January 2024 were recruited as FGR group(n=50),and healthy pregnant women who received prenatal examination during the same period were included in the control group(n=30).Both groups of pregnant women underwent ultrasound examination to monitor the blood flow parameters of uterine artery and fetal umbilical artery.All pregnant women performed routine prenatal examination in the same hospital,and complete blood samples were retained.The levels of PAPP-A and PLGF were detected by enzyme-linked immunosorbent assay,and the value of ultrasound blood flow parameters and serum factor levels in evaluating FGR was evaluated. Results:Compared with the control group,the higher parameters of uterine artery blood flow and umbilical artery blood flow(P<0.05),the lower levels of serum PAPP-A and PLGF(P<0.05)as well as the lower fetal biparietal diameter,head circumference,femoral length and abdominal circumference(P<0.05)were found in FGR group.The receiver-operating characteristic(ROC)curve for predicting FGR with different indications showed that the uterine artery and umbilical artery resistance index,the pulsatility index,the ratio of systolic maximum blood flow velocity to diastolic blood flow velocity,serum PAPP-A and PLGF levels had high predictive efficiency on FGR.The ratio of systolic maximum blood flow velocity to diastolic blood flow velocity of umbilical artery had the highest predictive efficiency,and when the cutoff value was>3.95,the Youden index,area under the curve(AUC),sensitivity and specificity for predicting FGR were 0.693,0.915,86.00%and 83.33%,respectively.The results of Pearson correlation analysis showed that the uterine artery and umbilical artery blood flow parameters of pregnant women in FGR group were negatively correlated with serum PAPP-A and PLGF levels,fetal biparietal diameter,head circumference,femoral length and abdominal circumference(P<0.05). Conclusions:Both ultrasound blood flow parameters and the levels of serum PAPP-A and PLGF can be used as effective indicators for the prediction of FGR.
