The predictive value of serum estradiol combined with progesterone detection on HCG day for IVF/ICSI-ET pregnancy outcomes of primary infertile women

Abstract
Objective:To investigate the predictive value of estradiol combined with progesterone on HCG day for the outcomes of assisted reproductive pregnancy among primary infertile women. Methods:A retrospective analysis of 251 patients with primary infertility treated by IVF/ICSI-ET in Qinzhou Maternal and Child Health Hospital from July 2021 to December 2023 was conducted and the patients were divided into the non-pregnant group(n=124)and the pregnant group(n=127)according to the pregnancy outcomes.The general characteristics,basic hormone levels,ovulation induction and embryo status of the two groups were compared.The influencing factors of assisted reproductive treatment in primary infertility women were analyzed with multivariate logistic regression analysis,and the receiver operating curve(ROC)was drawn to analyze the predictive value of estradiol combined with progesterone detection on HCG day in IVF/ICSI-ET pregnancy outcomes of primary infertility patients. Results:There was no significantly difference in body mass index,infertility years,total days of gonadotropin(Gn)use,Gn dose of initiation,endometrial thickness on HCG day,and basic levels of anti-Müllerian hormone(AMH),FSH,LH and progesterone(P)between the two groups(P＞0.05).The maternal age and P level on HCG day in the pregnancy group were significantly lower than those in the non-pregnancy group,and the basic estradiol(E2)level was significantly higher than that in the non-pregnancy group(P＜0.05).Multivariate logistic regression analysis showed that higher E2 level,the number of embryos transferred,lower maternal age,and P level on HCG day were protective factors for improving the pregnancy rate.The AUC of combined detection of E2 level and P level on HCG day in predicting the pregnancy outcomes of IVF/ICSI-ET among women with primary infertility was 0.604.The specificity was 71.8％,which was higher than those of E2 and P respectively. Conclusions:Serum E2 level combined with P level on HCG day has a certain predictive value for IVF/ICSI-ET pregnancy outcomes in women with primary infertility,which can be used as a reference index for clinical prediction of pregnancy rate in the patients mentioned above.关键词
