重型胎盘植入患者严重产后出血的危险因素分析

Abstract
Objective To analyze the risk factors of severe postpartum hemorrhage in patients with severe placenta accreta.Methods The clinical data of 207 patients with severe placenta accreta who underwent cesarean section in the Third Affiliated Hospital of Zhengzhou University from January 2019 to January 2024 were retrospectively analyzed.The amount of postpartum hemorrhage≥1500 ml was set as severe postpartum hemorrhage group,and<1500 ml was set as control group.The risk factors of severe postpartum hemorrhage in patients with severe placenta accreta were analyzed.Results Univariate analysis showed that parity,gestational diabetes mellitus,placenta accreta ultrasound score≥10 points,placental lacunae,blood flow signal of the posterior uterine wall of the placenta,retroplacental space,posterior muscular layer of the placenta,preoperative hemoglobin concentration and preoperative D-dimer concentration were associated with severe postpartum hemorrhage in patients with severe placenta accreta(P<0.05).Multivariate logistic regression analysis showed that ultrasound score of disc implantation≥10 points,gestational diabetes mellitus,low preoperative hemoglobin concentration,disappearance of placental posterior muscle layer,placental lacunae,and confluent placental lacunae with"boiling water sign"and blood flow signal in the posterior uterine wall of placenta suggested that the cross vessel sign were independent risk factors for severe postpartum hemorrhage in patients with severe placenta accreta(P<0.05).Conclusion There are many risk factors for severe postpartum hemorrhage in patients with severe placenta accreta.Preoperative risk prediction and assessment should be done to improve pregnancy outcomes.关键词
