DRG付费改革对医疗质量的影响研究
Impact of DRG payment reform on quality of medical care
摘要
Abstract
Objective To analyze the impact of DRG(Diagnosis-Related Group)payment reform on the quality of medical care in a tertiary medical institution.Methods Monthly data from January 2020 to December 2023 on 15 indicators in four dimensions—medical service capability,rational drug use,efficiency,and quality—were collected from the pilot reformed medical institutions.Entropy-weighted TOPSIS method and interrupted time series analysis(ITSA)were used to analyze the changing trends in various indicators before and after the reform.Results According to the entropy-weighted TOPSIS method of medical quality evaluation,the medical care quali-ty of the sample medical institution in September 2023 was at the optimal level in September 2023,while the eval-uation results in February 2020 were relatively poor.(Ci values:0.6810 and 0.1303 respectively),indicating that the medical care quality after the implementation of the DRG payment reform showed a trend of improvement and was better than that before the reform on the whole.The results of ITSA showed that the reform had a significant impact on the capacity of medical services,with significant improvements in the indicators of rational drug use and work efficiency,while the reform had a limited impact on the working quality of medical institutions.Conclusions Measures should be taken to effectively respond to the challenges brought about by the DRG payment reform by taking into account the characteristics and requirements of DRG payment and the actual situation of medical institu-tions,so as to continuously improve the DRG payment reform,and realize the steady improvement of the quality of medical care.关键词
