我国三级医院远程医疗建设及应用现状分析
Analysis of the Current Situation of Remote Medical Construction and Application in Tertiary Hospitals in China

Abstract
Objective To understand the construction and application status of telemedicine in tertiary hospitals in China,and provide reference for the future development of telemedicine in tertiary hospitals.Methods A multi-stage stratified sampling meth-od was adopted to select 100 tertiary hospitals in eastern,central,and western China to conduct online questionnaire surveys on hospitals'telemedicine capital investment,management modes,information technology construction,service development,and charging.Descriptive and comparative analyses were conducted by using constitutive ratios and chi-square test.Results 34.7％of tertiary hospitals invested 10,000 to 50,000 yuan in telemedicine network construction,and 33.7％invested less than 20,000 yuan in hardware facilities.The operation mode of telemedicine was mainly self-operation and maintenance mode,accounting for 74.7％,and was mainly managed by the medical department.The types of telemedicine operations carried out by tertiary hospitals were dominated by teleconsultation and tele-education,accounting for 93.7％and 58.9％,respectively,and the percentage of hospitals with an average annual service volume of more than 1,000 were 17.9％and 32.6％,respectively.Tele-diagnosis was dominated by tele-cardiology diagnosis,tele-imaging diagnosis,and tele-pathology diagnosis services,with the proportion of 53.7％,50.5％,and 42.1％,respectively,and the proportion of hospitals with an average annual service volume of more than 1,000 was 41.0％,26.3％,and 7.4％,respectively.There were statistical differences in the proportion of telemedicine services and volume of business in different regions(P＜0.05),and only 30.5％of the hospitals included telemedicine services in medi-cal insurance payments.Conclusion Patients in the central and western China have a strong demand for telemedicine services pro-vided by tertiary hospitals,but hospitals in the central region are less willing to invest in telemedicine construction.It is necessary to establish a standardized telemedicine service charging system,promote exchanges and cooperation among regional hospitals,promote telemedicine services,and mobilize high-quality medical resources in the eastern region to provide targeted assistance to the western region,so as to promote the implementation of the national hierarchical diagnosis and treatment system.关键词
