Abstract

Objective:Tumor-infiltrating lymphocytes(TILs)are critical immune cells within the tumor microenvironment,and their distribution and density are correlated with the prognosis and clinicopathological features of malignant tumors.This study aims to explore the distribution of CD8+TILs in colorectal cancer(CRC)and explore the clinicopathological and prognostic implications of their spatial patterns. Methods:A total of 211 CRC radical surgical specimens were collected.CD8 antibody immunohistochemistry was used to label and assess the distribution of TILs.Based on CD8+TIL distribution,tumors were classified into three immune phenotypes:Immune-desert,immune-excluded,and immune-inflamed.The relationships between CD8+TIL distribution patterns and clinicopathological characteristics,mismatch repair status,and prognosis were analyzed. Results:Among the 211 CRC cases,72 were immune-desert type,54 immune-excluded type,and 85 immune-inflamed type.CD8+TIL distribution types showed no significant association with general clinicopathological features(P>0.05).However,when combining immune-excluded and immune-inflamed types,the proportion of stage Ⅲ/Ⅳ cases was significantly lower than that in the immune-desert type(P<0.05).Kaplan-Meier survival analysis indicated that patients with the immune-desert type had the worst prognosis,while those with the immune-inflamed type had the best outcomes(P<0.05).CD8+TIL distribution was not directly associated with mismatch repair status,but when immune-excluded and immune-inflamed types were combined,the proportion of mismatch repair-deficient tumors was significantly lower compared to the immune-desert type(P<0.05).Both univariate and multivariate analyses identified CD8+TIL distribution patterns as an independent adverse prognostic factor in CRC. Conclusion:CD8+TILs exhibit distinct spatial distribution patterns in CRC.The immune-desert phenotype is associated with more advanced tumor stages,while the immune-inflamed phenotype predicts longer overall survival.Assessing CD8+TIL distribution may help predict mismatch repair status and patient prognosis in CRC.
