Abstract

Objective:In recent years,studies on endoplasmic reticulum stress(ERS)have increased,revealing its involvement in a variety of age-related diseases.Some scholars have confirmed a correlation between age-related hearing loss(ARHL)and ERS in animal and cell models.This study aims to explore the clinical correlation between serum C/EBP homologous protein(CHOP),glucose-regulated protein 78(GRP78),and fibroblast growth factor 21(FGF21)with mild to moderate ARHL. Methods:A total of 50 patients with mild to moderate ARHL were enrolled at the Affiliated Hospital of Xuzhou Medical University,serving as a hearing loss group,and 30 healthy individuals undergoing physical examination during the same period were included as a control group.All participants underwent clinical data collection and pure tone audiometry.According to audiometry results,subjects were divided into a normal(n=30,37.5%),a mild(n=29,36.25%),a moderate(n=14,17.5%),and a moderate-to-severe(n=7,8.75%)hearing loss groups.Serum CHOP,GRP78,and FGF21 levels were measured using enzyme-linked immunosorbent assay(ELISA).Spearman correlation analysis was used to assess the relationship between these serum markers and ARHL.Multivariate logistic regression identified influencing factors,and receiver operating characteristic(ROC)curve was used to evaluate the predictive value of the markers. Results:Compared to the control group,serum levels of CHOP,GRP78,and FGF21 were significantly elevated in the hearing loss group(all P<0.05).The proportion of patients with smoking history and poor sleep quality was also higher in the hearing loss group than that in the control group(Both P<0.05).Spearman analysis showed a positive correlation between CHOP levels and hearing loss severity(P<0.05).Logistic regression showed that sleep quality was a protective factor for ARHL(P=0.030),and GRP78 was a risk factor for ARHL(P=0.031).ROC analysis showed:CHOP had an area under the curve(AUC)of 0.677(sensitivity 68.0%,specificity 63.3%);GRP78 had an AUC of 0.812(sensitivity 66.0%,specificity 96.7%);FGF21 had an AUC of 0.828(sensitivity 62.0%,specificity 96.7%). Conclusion:Patients with mild to moderate ARHL may exhibit ERS dysregulation.Serum CHOP,GRP78,and FGF21 levels are correlated with the occurrence of ARHL,and FGF21 may provide a new idea for the early prevention of ARHL.
