Abstract

Objective:The analgesic efficacy of sufentanil varies significantly among individuals.This study aims to evaluate the influence of CYP2B6(CPB6)*4 and OPRM1 gene polymorphisms on postoperative intravenous analgesia with sufentanil in patients undergoing knee arthroscopic surgery. Methods:A total of 210 patients scheduled for elective anterior cruciate ligament reconstruction under combined spinal-epidural anesthesia were enrolled.Postoperative pain management was provided via intravenous patient-controlled analgesia.Prior to surgery,2 mL of peripheral venous blood was collected in anticoagulant tubes to detect the gene polymorphisms of CPB6*4 and OPRM1.Patients were grouped based on their genotypes:A/A,G/A,G/G for CPB6*4 rs2279343,and T/T,C/T for OPRM1 rs73568641.The following postoperative indicators were evaluated:Numerical Rating Scale(NRS)scores for pain,patient controlled analgesia(PCA)button press counts,sufentanil consumption,time to first onset of pain,and incidence of adverse events. Results:A total of 202 patients were included in the final analysis.For CPB6*4 rs2279343:76 were A/A,110 were G/A,and 16 were G/G.For OPRM1 rs73568641:194 were T/T and 8 were C/T.Compared to the G/A and A/A groups,patients in the G/G group had significantly lower NRS scores at postoperative day 7(both P<0.05);however,no significant differences were observed among the groups in NRS scores at days 1 and 3,PCA press counts,sufentanil consumption,time to first onset of pain,adverse events,postoperative hospital stay,or incidence of chronic pain at 3 months(all P>0.05).Compared to the T/T group,the C/T group had significantly higher NRS scores at postoperative days 1 and 3,more frequent PCA presses,and higher sufentanil usage(all P<0.05);however,no significant differences were found in NRS scores at day 7,pain onset time,adverse events,length of hospital stay,or 3-month chronic pain incidence(all P>0.05). Conclusion:Polymorphisms in the CPB6*4 and OPRM1 genes are genetic factors contributing to interindividual variability in postoperative intravenous sufentanil analgesia in patients undergoing knee arthroscopy.
