Abstract
Objective:Elderly patients with diabetes are at significantly higher risk for venous thromboembolism(VTE)due to chronic vascular lesions,hyperviscosity,and impaired immune function.However,there is currently a lack of comparative data on different VTE risk assessment scales in this population,leading to ongoing debate regarding tool selection in clinical practice.This study aims to evaluate the predictive value of the Padua Scale,Caprini Scale,and Autar Scale for VTE in elderly patients with diabetes.
Methods:A retrospective analysis was conducted on the clinical data of 262 hospitalized elderly diabetic patients treated between January 2019 and December 2023 at a single hospital,of whom 54 were diagnosed with VTE.VTE risk was assessed using the Padua Scale,Caprini Scale,and Autar Scale.The predictive performance of the 3 tools was compared using receiver operating characteristic(ROC)curves,and the area under the curve(AUC)was calculated.Optimal cutoff values for each scale were also determined.
Results:The AUCs for the Padua Scale,Caprini Scale,and Autar Scale were 0.622,0.713,and 0.634,respectively.The Caprini Scale demonstrated superior predictive performance compared to the other 2 scales.At the optimal cutoff point,the Caprini Scale showed the highest sensitivity,specificity,and Youden index.
Conclusion:Among elderly diabetic patients,the Caprini Scale offers greater accuracy in predicting VTE risk compared to the Padua Scale and Autar Scale.It is recommended as the preferred tool for VTE risk assessment in clinical settings.关键词
