医院—社区—家庭联动血糖健康管理的效果分析

Abstract
Objective To explore the application effect of hospital-community-family linkage blood glucose management.Methods A total of 72 cases of diabetic patients in a hospital from July to December 2023 were selected as the research object and randomly divided into control group and intervention group.The control group was given routine intervention,and the intervention group was given hospital-community-family linkage blood glucose management.The blood glucose level,self-management compliance and health management behavior level of the two groups were compared after intervention.Results After the intervention,the blood glucose levels and the improvements of health management behavior in the intervention group were better than those in the control group,and the Difference-in-Difference test showed that the net effect of the intervention was statistically significant(P<0.05),and the self-management compliance of the intervention group was higher than that of the control group(P<0.05).Conclusion The use of hospital community family linkage blood glucose management continuation program for the intervention of patients with diabetes had high value for strengthening the blood glucose control ability and self-management compliance of patients.关键词
