参芪降糖颗粒联合达格列净治疗糖尿病心肌病患者的临床疗效及对其MMP-9、TIMP-1水平的影响
Effects of Shenqi Jiangtang Granules Combined with Dapagliflozin on Diabetic Car-diomyopathy and Levels of MMP-9 and TIMP-1

Abstract
Objective To observe the effects of Shenqi Jiangtang Granules combined with dapagliflozin on diabet-ic cardiomyopathy(DCM)and levels of matrix metalloproteinase 9(MMP-9)and tissue inhibitor of matrix metalloprotein-ase 1(TIMP-1).Methods Eighty-two patients with DCM treated in Pingdingshan Traditional Chinese Medicine Hos-pital in Henan Province from March 2020 to March 2023 were selected and assigned with the random number table method into control and observation groups(n=41).Both groups received conventional treatment,with the control group receiving oral dapagliflozin on top of this,and the observation group receiving a combination of Shenqi Jiangtang Granules and oral dapagliflozin on top of conventional treatment,Both groups of patients were treated continuously for 12 weeks.The clinical efficacy was compared between the two groups.The blood glucose indicators[fasting plasma glucose(FPG),2-hour post-prandial blood glucose(2 h PG),and glycosylated hemoglobin(HbAlc)],TCM symptom scores,left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),MMP-9,and TIMP-1 were measured before and after treatment.Results After treatment,the levels of FPG,2 h PG,and HbA1c and TCM symptom scores in both groups declined(P＜0.05),and the observation group had lower blood glu-cose levels and TCM syndrome scores than the control group(P＜0.05).The treatment in both groups increased LVEF while decreasing LVEDD and LVESD(P＜0.05),and the observation group had higher LVEF and lower LVEDD and LVESD than the control group(P＜0.05).After treatment,both groups showed a decline in MMP-9 level and an eleva-tion in TIMP-1 level(P＜0.05),and the observation group had lower MMP-9 level and higher TIMP-1 level than the control group(P＜0.05).The total response rate in the observation group was 92.68％(38/41),which was higher than that(78.05％,32/41)in the control group(P＜0.05).Conclusion The combination of Shenqi Jiangtang Granules and dapagliflozin can effectively alleviate symptoms and ventricular remodeling and improve the cardiac pumping capacity in DCM patients by regulating the expression of TIMP-1 and MMP-9 and maintaining the stability of the extracellular ma-trix environment in myocardial cells.关键词
