Abstract
Objective To investigate the correlation of serum soluble transferrin receptor(sTfR)and soluble fibrinogen-like protein 2(sFGL2)levels with cardiac function indicators and ventricular remodeling in elderly patients with chronic heart failure(CHF).Methods From June 2022 to September 2023,122 elderly CHF patients treated at the Second People's Hospital of Zhangjiagang City were collected as the CHF group.According to the New York Heart Association(NYHA)cardiac function grading standards,the CHF group was divided into Class Ⅱ(40 cases),Class Ⅲ(45 cases),and Class Ⅳ(37cases).Additionally,109 healthy individuals undergoing routine physical examinations were included as the control group.Serum sTfR and sFGL2 levels were measured using enzyme-linked immunosorbent assay.Cardiac function and ventricular remodeling indicators were assessed using echocardiography,including cardiac output(CO),left ventricular ejection fraction(LVEF),left ventricular posterior wall thickness(LVPWT),left ventricular end-diastolic diameter(LVEDD),left atrial diameter(LAD),left ventricular mass index(LVMI),and left ventricular remodeling index(LVRI).Pearson correlation analysis was used to evaluate the association between serum sTfR,sFGL2 levels and cardiac function and ventricular remodeling parameters.Results Compared to the control group,serum sTfR in the CHF group was significantly increased(t=12.667,P<0.01),while the sFGL2 level was significantly reduced(t=10.608,P<0.01).The LVPWT,LVEDD,LAD,and LVMI in the CHF group were significantly higher than those in the control group(t=6.684,12.047,9.474,6.687;P<0.01),while CO,LVEF,and LVRI were significantly lower(t=5.091,8.552,8.562;P<0.01).With the increase in heart function classification,sTfR,LVPWT,LVEDD,LAD,and LVMI levels increased(F=38.538,60.125,65.663,42.303,34.412;P<0.01),while sFGL2,CO,LVEF,and LVRI levels decreased(F=48.900,69.286,29.240,26.350;P<0.01).In elderly CHF patients,serum sTfR levels were positively correlated with LVPWT,LVEDD,LAD,and LVMI(r=0.609,0.586,0.562,0.545;P<0.01),but negatively correlated with CO,LVEF,and LVRI(r=-0.651,-0.696,-0.710;P<0.01).Conversely,sFGL2 levels were negatively correlated with LVPWT,LVEDD,LAD,and LVMI(r=-0.636,-0.581,-0.594,-0.511;P<0.01),but positively correlated with CO,LVEF,and LVRI(r=0.652,0.649,0.746;P<0.01).Conclusion In elderly CHF patients,elevated serum sTfR levels and reduced sFGL2 levels are associated with cardiac function indicators and ventricular remodeling.These biomarkers may serve as serum markers for evaluating cardiac function indicators and ventricular remodeling in elderly CHF patients.关键词
