Abstract
Objective Malignant otitis externa(MOE)is an invasive and progressive infection of the external auditory canal and skull base,which mostly affects patients with diabetes mellitus(DM),the elderly and immunocompromised individuals.This investigation systematically describes clinical characteristics of patients with MOE and its correlation between DM.Methods Established medical databases were searched for articles assessing MOE in patients with DM.Pathogenesis,signs and symptoms,treatment algorithms,complications and association with DM were analyzed.Results Forty-seven studies were eligible with a total sample size of 10,672 cases.The overall prevalence of DM in the MOE cohort was 63%.The average duration of DM was 12.24±6.91 years characterized by a pooled HbA1c of 8.62±2.49%.The most common complications were otalgia(96.1%),external auditory canal swelling(75.8%)and otorrhea(62.3%).Pseudomonas aeruginosa was the most frequently isolated microorganism(71.6%).Fluoroquinolones and 3rd-generation cephalosporins were the preferred antibiotics.The pooled rate of resolution with antibacterial drugs was 75.9%.In addition to medications,24.6%of the affected individuals required debridement.The overall mortality rate was 1.6%.Conclusion MOE is associated with poorly controlled DM from an evidence-based perspective,which has certain guiding value for the treatment for patients with MOE and comorbid DM in clinical practice.关键词
