Abstract
Objective To report the status of eustachian tube function after tympanoplasty in patients with chronic suppurative otitis media(CSOM),and its impacts on clinical outcomes.Methods Data from 139 patients with CSOM(141 ears)admitted to the Department of Otorhinolaryngology,Affiliated Hospital of Zunyi Medical University from November 2021 to September 2022 were retrospective collected as the study group,and 36 patients who underwent tympanic membrane repair for traumatic tympanic membrane perforation(36 ears)were used as the control.All operation were conducted under general anesthesia and patients were followed up on a regular basis.Postoperative eustachian tube function was assessed at 3 months after surgery,in addition to otoendoscopy examination,Valsalva test and the ETDQ-7 questionnaire.The influence of postoperative eustachian tube function on the clinical outcomes was further analyzed.Results The rate of tympanic membrane healing was 96.5%in the study group,with both postoperative pure tone threshold average(PTA)(31.80±14.84 dB HL)and air-bone conduction gap(ABG)(18.67±10.12 dB)improved,compared to before surgery(P＜0.01).The postoperative eustachian tube score in the study group(1.36±1.46)was higher than in the control group(0.75±1.22)(P＜0.05).In the study group,the rate of tympanic membrane healing was 97.4%(112/115 ears)in patients with adequate eustachian tube function and 92.3%(24/26 ears)in those with dysfunctional eustachian tubes,respectively,(P＞0.05).Postoperative PTA was at 29.23±13.59 dB HL(13.48±8.52 dB improvement)and ABG at 17.10±9.46 dB(11.97±8.91 dB improvement)in the former,and at 43.19±15.03 dB HL(6.19±5.39 dB improvement)and 25.62±10.50 dB(4.92±5.40 dB improvement)in the latter,respectively,(P＜0.01).Conclusion Some patients with chronic suppurative otitis and no eustachian tube issues can suffer from eustachian tube dysfunction following tympanoplasty surgeries.The status of eustachian tube function can affect hearing improvement after operation.关键词
