短期多元整合声对慢性主观性耳鸣患者的疗效及对大脑皮层神经活动的影响
Effects of Short-Term Multi-Component Integrated Sounds on Patients with Chronic Subjective Tinnitus and on Cerebral Cortical Neural Activities

Abstract
Objective To evaluate the efficacy of short-term multiple integrated sounds(MIS)in the treatment of chronic subjective tinnitus,and their effects on cerebral cortical neural activities in patients with chronic subjective tinnitus based on oxygenated hemoglo-bin(HbO),as well as their correlation.Methods Patients with chronic subjective tinnitus(n=180)were prospectively selected from the Otolaryngology Department,Affiliated Hospital of North Sichuan Medical College from January 2023 to March 2023,and randomly divided into a MIS treatment group and a control group to be treated with traditional narrowband noise(90 in each).Treatment session was 15 min.Tinnitus Handicap Inventory(THI)scores and visual analogue scale(VAS)scores were used to evaluate tinnitus severity before and after treatment.Functional near-infrared spectroscopy(FNIRS)was used to determine hemoglobin oxygenation at frontal polar cortex(FPC-HbO),dorsolateral prefrontal cortex(DPFC-HbO),temporal cerebral cortex(TL-HbO),occipital cerebral cortex(OL-HbO)and parietal cerebral cortex(PL-HbO).Results Before treatment,there was no significant difference in THI and VAS scores between the two groups(P>0.05).After treatment,the THI score showed no significant difference compared to before treatment or between the two groups(P>0.05).The post-treatment VAS score decreased in both groups,more so in the MIS group(P<0.05).Post-treatment DFC-HbO and TL-HbO increased in both groups(P<0.05),but FPC-HbO decreased(P<0.05),while PL-HbO and OL-HbO showed no significant change(P>0.05).FPC-HbO change in the MIS group was greater than in control group(P<0.05).The FPC hemodynamic topographic map showed no significant before and after treatment change in the control group,but reduced HbO concentration in the MIS group.The change of FPC-HbO in the MIS group was positively correlated with VAS score changes(r=0.540,P<0.05).Conclusion Short-term MIS can change neuronal activities in the frontal cortex and reduce perceived tinnitus in patients with chronic subjective tinnitus,and the appear to be correlated.关键词
