Qualitative studies on ICU nurses'clinical practice experiences in caring for delirium patients:A Meta-integration

Abstract
Objective:Delirium remains highly prevalent among critically ill patients,and nurses's caregiving experiences directly influence the effectiveness of delirium management.This study aims to systematically evaluate and synthesize qualitative studies on Intensive Care Unit(ICU)nurses'clinical experiences of in caring for patients with delirium,and to provide reference for improving delirium care strategies. Methods:A comprehensive literature search was conducted in databases including CNKI,WanFang,VIP,CBM,PubMed,Web of Science,CINAHL,Cochrane Library,and Embase for qualitative studies on ICU nurses'clinical experiences with delirium patients,from inception to June 2024.Meta-integration using an aggregative integration method was applied to extract and integrate findings from the original results. Results:A total of 14 qualitative studies were included,yielding 40 individual findings that were categorized into 12 themes and synthesized into 3 overarching results.First,the barriers to delirium management included:Difficulties in early identification,insufficient knowledge and training,lack of awareness,conflicts among healthcare providers and patients or families,negative emotions among nurses,and obstacles during assessment.Second,facilitators for effective management were identified as nurses'knowledge-belief-practice alignment,interdisciplinary collaboration,family involvement,and early mobilization.Third,recommendations included establishing standardized nursing protocols for delirium and developing subtype-specific management models. Conclusion:Clinical managers should prioritize early identification of ICU delirium and establish efficient screening and assessment workflows.Individualized interventions should be implemented based on delirium subtypes.Additionally,nurses'caregiving experiences and emotional well-being should be supported to enhance the overall capacity for delirium management in critical care settings.关键词
