Systematic review of the measurement properties of needs assessment tools for stroke patients based on COSMIN

Abstract

Objective:Stroke is a leading cause of death and disability among adults in China.Stroke-related sequelae often impair patients'ability to meet their own needs,seriously affecting their quality of life.Utilizing scientific assessment tools to accurately and rapidly evaluate patient needs is essential for delivering timely and targeted medical and nursing interventions,thereby improving patient outcomes.Although numerous tools are currently available for assessing the needs of stroke patients,a comprehensive and standardized comparison of these instruments is lacking,making it difficult for clinicians to choose appropriate tools.This study systematically reviews the available needs assessment tools for stroke patients based on the Consensus-Based Standards for the Selection of Health Measurement Instruments(COSMIN)to provide evidence-based recommendations for clinical practice. Methods:Databases including CNKI,Wanfang,VIP,Chinese Biomedical Literature Database,PubMed,Web of Science,Cochrane Library,and CINAHL were searched.Included tools were evaluated using the COSMIN checklist,and final recommendations were formed. Results:A total of 14 studies were included,encompassing 10 different needs assessment tools for stroke patients:Professional Care Needs Tool for Home-Based Stroke Patients,Care Needs Questionnaire for Ischemic Stroke Patients,Rehabilitation Needs Scale for Stroke Patients,Timely Needs Survey for Stroke Patients,Continuing Care Needs Questionnaire for Elderly Stroke Patients,Rehabilitation Needs Questionnaire for Stroke Patients,Sydney Psychosocial Reintegration Scale(SPRS),Handicap Assessment Questionnaire:The Impact on Participation and Autonomy(IPA),Longer-term Unmet Needs after Stroke(LUNS),and Poststroke Checklist(PSC).Evaluation based on the COSMIN showed that all tools had"doubtful"methodological quality in content validity,with measurement properties rated as"adequate""inadequate"or"unclear".Structural validity was unreported in SPRS,PSC,and parts of LUNS,while the Rehabilitation Needs Questionnaire had"very good"methodological quality and"adequate"measurement properties;others were mostly"adequate"or"good".Criterion validity was mostly unreported except for SPRS and LUNS,which were rated"poor"and"inadequate".Internal consistency was"very good"and"adequate"for most tools,except PSC("not reported")and LUNS("doubtful"and"not reported").Reliability was best for the Home-Based Stroke Care Needs Tool("good"and"adequate")but varied in LUNS and was largely unreported in other tools.Measurement error data were limited or unclear across most tools.Among the instruments,the Rehabilitation Needs Questionnaire provided the most comprehensive assessment across multiple domains with a manageable item count,while the LUNS scale was the most widely applied for assessing unmet needs. Conclusion:The Rehabilitation Needs Questionnaire for Stroke Patients and the LUNS scale have undergo relatively comprehensive psychometric evaluation and demonstrated good reliability and validity,making them provisionally recommended tools.However,further validation of other psychometric properties is still necessary.
