四维自动左室定量分析技术评价梗阻性肥厚型心肌病合并阵发性心房颤动患者左室收缩功能
Assessment of left ventricular systolic function in hypertrophic obstructive cardiomyopathy patients combined with paroxysmal atrial fibrillation by four-dimensional automatic left ventricular quantitative analysis

Abstract
Aim:To assess the left ventricular systolic function in hypertrophic obstructive cardiomyopathy(HOCM)patients combined with paroxysmal atrial fibrillation(PAF)by four-dimensional automatic left ventricular quantitative analy-sis(4D Auto LVQ).Methods:A total of 29 HOCM patients complicated with PAF(HOCM with PAF group)treated in the First Affiliated Hospital of Zhengzhou University were collected.Additionally,29 HOCM patients without PAF(HOCM group)and 29 healthy volunteers(normal control group)were selected as comparative cohorts.The conventional ultrasound parameters were collected,and the left ventricular myocardial mechanical parameters,including global longitudinal strain(GLS),global circumferential strain(GCS),global radial strain(GRS),and global area strain(GAS),were obtained using 4D Auto LVQ technology.The differences in the above mentioned parameters were compared among the 3 groups.Results:Compared with normal control group,the HOCM with PAF group and HOCM group exhibited significant increases in left ventricular maximal wall thickness(LVMWT),left atrial diameter(LAD),and E/e'levels.Additionally,a significant de-crease was observed in left ventricular end diastolic diameter(LVEDD)(P<0.05).Compared with the HOCM group,the HOCM with PAF group exhibited a significant increase in LAD(P<0.05).There were no significant differences in left ven-tricular ejection fraction(LVEF)or E/A among the 3 groups(P>0.05).Compared with normal control group,the HOCM with PAF group and HOCM group exhibited decreased GLS,GRS,and GAS levels(P<0.05).Additionally,the HOCM with PAF group demonstrated lower GLS,GRS,and GAS levels compared with the HOCM group(all P<0.05).Furthermore,GCS in the HOCM with PAF group was significantly lower than both the HOCM group and normal control group(P<0.05).Conclusion:The 4D Auto LVQ technique can effectively assess the impairment of left ventricular systolic function in HOCM patients combined with PAF,providing valuable evidence for early clinical intervention.关键词
