医养结合服务模式下社区居家老年人健康相关生命质量的影响因素分析
Analysis on influencing factors of health-related quality of life among the community-dwelling elderly under the integrated medical and elderly care service mode

Abstract
Aim:To explore the factors influencing health-related quality of life(HRQoL)among the community-dwell-ing elderly under the integrated medical and elderly care service mode.Methods:A multi-stage stratified cluster random sampling method was used to conduct an HRQoL survey among 1 376 community-dwelling elderly receiving integrated medi-cal and elderly care services in 4 cities:Suzhou,Guangzhou,Qingdao,and Jinan.The EQ-5D-3L scale was used to assess and calculate health utility values.The health utility values of the elderly with different characteristics were compared,and a Tobit regression model was employed to analyze the factors influencing HRQoL among the community-dwelling elderly.Re-sults:The health utility value of the 1 376 community-dwelling elderly was 0.790(0.510,0.960).Higher health utility val-ues were observed among the elderly who drank alcohol 1-2 times a month(β=0.051,95％CI:0.005-0.097),had sleep durations of＜4 h/d(β=0.137,95％CI:0.065-0.209),4～＜6 h/d(β=0.131,95％CI:0.061-0.202),and 6～＜8 h/d(β=0.115,95％CI:0.040-0.190),and those with pension insurance(β=0.057,95％CI:0.026-0.088).Lower health utility values were observed among the elderly aged≥ 90(β=-0.123,95％CI:-0.178--0.069),former smok-ers who had quit(β=-0.064,95％CI:-0.102--0.026),those who went out twice a month(β=-0.170,95％CI:-0.230--0.110)or less frequently(β=-0.332,95％CI:-0.370--0.294),and those with multiple chronic con-ditions(β=-0.061,95％CI:-0.086--0.035).Conclusion:The HRQoL of the community-dwelling elderly under the integrated medical and elderly care service mode is relatively poor.It is necessary for the government to improve pension in-surance policies,and give corresponding health guidance.关键词
