Abstract
Objective:To investigate the application value of paraspinal muscular approach and paraspinous approach in patients with spinal fractures.Methods:Data of 94 patients with spinal fractures admitted to Miyun Hospital of Peking University First Hospital from September 2022 to August 2024 were retrospectively analyzed,and they were divided into a study group and a control group,with 47 patients in each group,based on different approaches.The study group used the paraspinal muscular approach,while the control group used the paraspinous approach.Both groups were followed up for 4 weeks after surgery.The surgical related indicators,pain level,dysfunction level,skeletal muscle trauma,and the occurrence of adverse events between the two groups were compared.Results:The surgical time and postoperative bed rest time of the study group were shorter than those of the control group,and the intraoperative blood loss was less than that of the control group,with statistical significance(P<0.05).Four weeks after surgery,the Pain Visual Analogue Scales and Oswestry Disability Index score in the study group were lower than those in the control group,with statistical significance(P<0.05).Three days after surgery,the levels of myoglobin and creatine kinase in the study group were lower than those in the control group,with statistical significance(P<0.05).The incidence of adverse events in the study group was 6.38%,which was lower than the 10.64%in the control group,but the difference between the two groups was not statistically significant(P>0.05).Conclusion:Compared with the paraspinal approach,the paravertebral intermuscular space approach can effectively shorten the operative time and postoperative bed time of patients with spinal fracture,reduce the amount of intraoperative blood loss,alleviate postoperative pain and improve functional dysfunction,and reduce the occurrence of skeletal muscle injury and postoperative complications.关键词
