穿通支静脉激光闭合术与溃疡旁静脉轮辐状激光闭合术治疗下肢静脉溃疡效果对比观察
Comparison of efficacy between laser closure of perforating veins and radial laser closure of veins around the ulcer in the treatment of lower extremity venous ulcers

Abstract
Objective To compare and observe the effects of laser closure of perforating veins and radial laser clo-sure of veins adjacent to ulcers in the treatment of lower limb venous ulcers.Methods Sixty patients with lower limb ve-nous ulcers were randomly assigned into the experimental group and control group,with 30 patients in each group.Patients in the experimental group received laser closure of perforating veins,while patients in the control group received laser clo-sure of the veins adjacent to the ulcer in a radial pattern.Patients of both groups were regularly followed up after surgery to record the healing time of lower limb venous ulcers and the occurrence of postoperative complications.Results All 60 pa-tients successfully completed the surgery,of whom 58 patients did not show significant swelling,incision infection,or skin necrosis after surgery,and the ulcer wounds healed within 28 days.One patient in the experimental group had suspected eczema in the area around the ulcer after surgery.After treatment,the symptoms improved and the ulcer healed after 35 days.One patient in the control group had the original ulcer healing within 2 weeks after surgery,but there was skin infec-tion and ulceration near the ulcer.After treatment,the ulcer healed after 42 days.The patients did not show any hidden nerve damage or deep vein thrombosis after surgery.The experimental group consisted of 19 patients with ulcer area≥3 cm2,and the ulcer healing time was(16.50±6.62)days;the control group consisted of 21 patients with ulcer area≥3 cm2,and the ulcer healing time was(18.40±6.18)days;there was significant difference between these two groups(all P<0.05).The experimental group consisted of 11 patients with ulcer area less than 3 cm2,and the ulcer healing time was(8.23±3.15)days;the control group consisted of 9 patients with ulcer area less than 3 cm2,and the ulcer healing time was(9.50±3.32)days;there was no significant difference between these two groups(all P>0.05).Conclusion Com-pared with the radial laser closure of the veins adjacent to the ulcer,patients with lower limb venous ulcers with an ulcer ar-ea of≥3 cm2 who receive laser closure of the perforating branch vein have faster ulcer healing.关键词
