Abstract
Objective:To investigate the long-term efficacy and influencing factors of laparoscopic Miles surgery in patients with low rectal cancer.Methods:The data of 120 patients with low rectal cancer admitted to Jingtai County Hospital of Traditional Chinese Medicine and the Second Hospital of Lanzhou University from April 2020 to April 2021 were retrospectively analyzed.All patients were treated with Miles laparoscopic surgery.The recurrence or distant metastasis of cancer during 3-year follow-up were analyzed and divided into good treatment group(n=89)and poor treatment group(n=31)according to whether there was recurrence or distant metastasis.Data of patients were collected,including gender(male,female),age(≥60 years old,<60 years old),body mass index(≥24 kg/m2,<24 kg/m2),education level(high school and below,college or above),marital status(in marriage,unmarried or divorced),tumor diameter(≥5 cm,<5 cm),the degree of tumor differentiation(low differentiation,medium differentiation/high differentiation),whether neoadjuvant chemotherapy was received(no,yes),the level of embryonic cancer antigen(≥13.6 U/L,<13.6 U/L),and the number of lymph node metastasis(≥4,<4).The factors influencing the efficacy of laparoscopic Miles in the treatment of low rectal cancer were analyzed.Results:Among the 120 patients,17 cases of cancer recurred,the recurrence rate was 14.17%;14 cases had distant metastasis,the metastasis rate was 11.67%.Univariate analysis showed that tumor diameter,tumor differentiation degree,whether receiving neoadjuvant chemotherapy,embryonic cancer antigen level,and the number of lymph node metastases were influencing foctors of the efficacy of laparoscopic miles surgery in patients with low rectal cancer(P<0.05).Logistic regression analysis showed that tumor diameter≥5 cm,poorly differentiated tumor degree,no neoadjuvant chemotherapy,embryonic cancer antigen level≥13.6 U/L,and number of lymph node metastasis≥4 were independent risk factors affecting the long-term efficacy of patients undergoing Miles laparoscopic surgery for low rectal cancer(P<0.05).Conclusion:Whether the long-term efficacy of laparoscopic Miles surgery in the treatment of low rectal cancer curative effect is ideal or not is related to risk factors like the tumor diameter≥5 cm,the degree of tumor differentiation,the absence of neoadjuvant chemotherapy,the level of embryonic cancer antigen≥13.6 U/L,the number of lymph node metastasis≥4.关键词
