激光定位装置辅助192Ir插植治疗周围型非小细胞肺癌的应用研究
Clinical observation of 192Ir implantation assisted by laser positioning device in treatment of peripheral non-small-cell lung cancer

Abstract
Objective To investigate the guiding value of a self-made laser positioning device in assisting CT-guided 192Ir stereotactic ablative brachytherapy(SABT)for peripheral non-small-cell lung cancer(NSCLC).Methods A retrospective analysis was conducted on 60 patients with peripheral NSCLC who underwent CT-guided 192Ir SABT at the Affiliated Hospital of Southwest Medical University from January 1st,2022,to December 31st,2023.Among them,30 patients who used a self-made laser positioning device to assist in inserting interstitial needles were the laser-assisted group,and 30 patients undergoing manual insertion of interstitial needles were the freehand implantation group.All patients had the position and depth of the interstitial needles adjusted under CT guidance,and CT images meet-ing clinical requirements were imported into the Oncentra Brachy TPS.Physicians delineated target volumes and organs at risk within this system,physicists completed the brachytherapy plan,and technicians implemented SABT.The clinical efficacy of SABT between the two groups was assessed.Results The postoperative dosimetric parameters including V100,D100,D90,conformity index(CI),and homogeneity index(HI)were all higher in the laser-assisted group than those in the freehand implantation group(all P<0.05).The postoperative V150 showed no significant difference between the two groups(P>0.05).The preoperative and postoperative dosimetric parameters including V150,V100,D100,D90,CI,and HI showed no significant difference in the laser-assisted group(all P>0.05),while those in the freehand implanta-tion group were significantly different(all P<0.05).The laser-assisted group required(4.07±1.31)CT scans,(20.37±2.78)min to reach satisfactory needle positions,and(3.70±1.53)needles per implantation,whereas the freehand implantation group required(5.07±1.34)CT scans,(23.50±2.99)min,and(4.87±1.48)needles.The differences between the two groups were statistically significant(all P<0.01).Conclusions In SABT for peripheral NSCLC patients,the use of the self-made laser positioning device for assistance in implantation has significant dosimetric advantages,ensuring consistency between preoperative and postoperative radiotherapy plans in SABT.The device enables physicians to operate more purposefully,reduces the number of needles and operation time,avoids secondary injuries caused by frequent needle adjustments during manual implantation,and alleviates suffering.关键词
