不同体重指数多囊卵巢综合征患者的临床特征分析
Analysis of clinical characteristics of different body mass index in patients with polycystic ovary syndrome

Abstract
Objective To compare the differences of glucose and lipid metabolism parameters,endocrine hormone levels,liver function and other indicators in polycystic ovary syndrome(PCOS)patients with different body mass index(BMI).Methods A retrospective analysis was conducted on the clinical data of 122 patients with PCOS diagnosed in Sir Run Run Hospital,Nanjing Medical University from April 2021 to December 2023,and they were divided into low weight group(n=13,BMI<18.5 kg/m2),normal weight group(n=57,18.5 kg/m2≤BMI<24.0 kg/m2),overweight group(n=25,24.0 kg/m2≤BMI<28.0 kg/m2),and obese group(n=27,BMI≥28.0 kg/m2)according to different BMI.The glucose and lipid metabolism indexes,endocrine hormone levels,liver function and other indicators(homeostasis model assessment of insulin resistance[HOMA-IR],luteinizing hormone/follicle stimulating hormone[LH/FSH],free androgen index[FAI])were compared among the four groups.Results FAI and HOMA-IR in the obese and overweight groups were higher than those in the normal weight and low weight groups(P<0.05).FAI and HOMA-IR in the obese group were higher than those in the overweight group(P<0.05).Sex hormone binding globulin in the obese and overweight groups was lower than that in the normal weight and low weight groups(P<0.05).Sex hormone binding globulin in the obese group was lower than that in the overweight group(P<0.05).The levels of fasting blood glucose,fasting insulin,triglyceride,total cholesterol,low density lipoprotein,alanine amino-transferase,and aspartate transferase in the obese group were higher than those in the normal weight group and the low weight group(P<0.05).The levels of fasting insulin and triglyceride in the obese group were higher than those in the overweight group(P<0.05).The levels of fasting blood glucose,fasting insulin,triglyceride,low density lipoprotein,and alanine amino-transferase in the overweight group were higher than those in the normal weight group and the low weight group,and the total cholesterol level was higher than that in the low weight group(P<0.05).The levels of LH and LH/FSH in the obese group were lower than those in the normal weight group,and the levels of thyroid stimulating hormone in the obese group were higher than those in the normal weight group and the low weight group(P<0.05).Conclusion Obese and overweight PCOS patients are more likely to have abnormal glucose and lipid metabolism,insulin resistance,increased FAI,and low expression of sex hormone binding globulin.Obese PCOS patients have higher risk of metabolic syndrome,abnormal liver function,and abnormal thyroid function.Normal weight and low weight PCOS patients are more common to have high LH and LH/FSH≥2.In the clinical management of PCOS patients with different BMI,individualized management measures and treatment plans should be emphasized to improve the diagnosis,treatment and prognosis.关键词
