Abstract
Objective:To investigate the differences in complications between ultrasound-guided and anatomical landmark percutaneous dilatational tracheostomy(PDT)in critically ill patients.Methods:241 patients who underwent bedside PDT in the Intensive Care Unit of the First Affiliated Hospital of Shantou University Medical College from July 2020 to August 2024 were retrospectively selected as study subjects.The patients were divided into anatomical landmark group(n=149)and ultrasound-guided group(n=92)according to the different surgical operation methods.Gender,age,preoperative coagulation function,preoperative platelet level,and intraoperative and postoperative complications were compared between the two groups.Results:The differences between the two groups of patients in terms of gender,age,preoperative coagulation function and preoperative platelet level were not statistically significant(P>0.05).The differences between the two groups were not statistically significant in terms of the complication rates of subcutaneous emphysema,pneumothorax,bleeding requiring intervention,intraoperative hypoxemia,poor wound healing,and tracheal stenosis(P>0.05).However,the overall complication rate in the ultrasound-guided group was significantly lower than that in the anatomical landmark group,and the difference was statistically significant(4.3%vs.12.8%,P=0.031).Conclusion:Ultrasound-guided PDT reduces the overall complication rate.关键词
