血糖和炎症指标水平对机械通气重症肺炎患者预后的预测价值分析
Analysis of the Prognostic Value of Blood Glucose and Inflammation Indexes Levels on the Prognosis of Severe Pneumonia Patients with Mechanical Ventilation

Abstract
Objective:To explore the prognostic value of glycosylated hemoglobin(HbA1c)and inflammation index indexes levels in severe pneumonia patients with mechanical ventilation.Method:A total of 81 patients with severe pneumonia admitted to the intensive care unit(ICU)in Shangrao Municipal Hospital from January 2022 to December 2023 were selected as the study objects,and they were divided into death group(n=27)and survival group(n=54)according to the prognosis of the patients within 1 month.Within 24 h after admission,the levels of hemoglobin(Hb),white blood cell count(WBC),neutrophil percentage(NEUT%)and platelet count(PLT)were detected by full-automatic blood analyzer,the HbA1c level was detected by full-antomatic glycosylated hemoglobin analyzer,serum interleukin-6(IL-6)level was detected by enzyme linked immunosorbent assay,serum procalcitonin(PCT)level was detected by immunofluorescence method,and serum hypersensitive C reactive protein(hs-CRP)level was detected by immunonephelometry.The predictive value of NEUT,HbA1c,PCT,hs-CRP and IL-6 levels for death in pneumonia patients with mechanical ventilation were analyzed using receiver operating characteristic(ROC)curve.Result:There were no significant differences in Hb,WBC and PLT levels between survival group and death group(P>0.05).The levels of NEUT%and HbA1c in the death group were significantly higher than those in the survival group,the differences were statistically significant(P<0.05).The levels of serum PCT,hs-CRP and IL-6 in the death group were significantly higher than those in the survival group,the differences were statistically significant(P<0.05).The combined detection of NEUT,HbA1c,PCT,hs-CRP and IL-6 levels predicted the death of severe pneumonia patients with mechanical ventilation with an area under the curve(AUC)of 0.904,sensitivity of 82.00%and specificity of 74.07%.Conclusion:The combined detection of NEUT%,HbA1c,PCT,hs-CRP and IL-6 levels has a high predictive value for the prognosis of severe pneumonia patients with mechanical ventilation.关键词
