吕海江辨治干眼经验探析
Analysis on LV Haijiang's Experience in Differentiating and Treating Dry Eye

Abstract
Dry eye disease(DED)is a prevalent chronic ocular surface disorder with an increasing incidence,severely impacting patients'quality of life.Its pathogenesis is primarily associated with ocular surface inflammatory responses,leading to reduced tear film stability and damage to ocular surface epithelial cells.LV Haijiang emphasizes that although dry eye manifests in the eyes,it is closely linked to the lungs,liver-kidney system,and spleen-stomach according to Traditional Chinese Medicine(TCM)theory.From a TCM perspective,its essence lies in the dysfunction of the five viscera,depletion of essential Qi,essence,and body fluids,resulting in malnourishment of the ocular orifices.In clinical pattern differentiation,He classifies dry eye into multiple syndrome patterns:Lung Yin deficiency,where insufficient body fluids fail to ascend and moisten the eyes;Liver-kidney deficiency,leading to inadequate nourishment of the eyes by essence and blood;Spleen-stomach damp-heat,where turbid pathogens obstruct the upward flow of nutrients;Liver channel stagnation-heat,causing heat pathogens to consume Yin fluids.Corresponding therapeutic strategies are employed for these patterns,including nourishing lung Yin,tonifying liver and kidneys,clearing damp-heat,and soothing liver heat to brighten the eyes.A dual-approach regimen is advocated,combining internal medical therapy to regulate visceral functions with external medical fumigation for direct ocular application.Notably,He underscores the importance of holistic management:maintaining emotional equilibrium,avoiding excessive stress,adopting balanced diets,adhering to regular sleep cycles,and practicing ocular hygiene to prevent recurrence.This paper synthesizes his distinctive insights and extensive clinical experience,offering valuable perspectives for advancing evidence-based dry eye management.关键词
