Abstract
Objective:To study the effect of low molecular weight heparin calcium(LMWH)as adjuvant therapy on inflammation and coagulation in patients with diabetic foot(DF).Methods:A total of 46 DF patients admitted to a hospital from January 2021 to January 2024 were selected as the research subjects and divided into an observation group and a control group using the random number table method,with 23 cases in each group.All patients were given lifestyle guidance,hypoglycemic treatment,anti-infection treatment and other symptomatic treatment.The control group was given debridement treatment+local application of bovine basic fibroblast growth factor,the observation group was given subcutaneous injection of low molecular weight heparin calcium injection in addition to the control treatment.After 1 month of treatment,the two groups were compared in terms of glucose metabolism control[fasting blood glucose(FPG),2 hour postprandial blood glucose(2hPG),glycosylated hemoglobin(HbA1c)],inflammatory factor levels[vascular cell adhesion molecule-1(VCAM-1),C-reactive protein(CRP),interleukin-6(IL-6)],coagulation[fibrinogen(FIB),activated partial thromboplastin time(APTT),prothrombin time(PT)],foot indexes(long diameter and short diameter of ulcers,wound granulation,blood flow velocity of dorsalis pedis artery)and clinical efficacy.Results:After one month of treatment,in the two groups,the levels of FPG,2hPG,HbA1c were decreased,and lower in the observation group than in the control group(P<0.05).VCAM-1,IL-6,CRP and FIB were all decreased,and lower in the observation group than in the control group(P<0.05).APTT and PT were prolonged,and longer in the observation group than in the control group(P<0.05).The long and short diameters of ulcers in the observation group were smaller than those in the control group,and the proportion of fresh granulation tissue in the wound and blood flow velocity of dorsal pedis artery of foot in the observation group were higher than those in the control group(P<0.05).Moreover,the total effective rate of the observation group(95.65%)was higher than that of the control group(73.91%,P<0.05).Conclusion:Low molecular weight heparin calcium as adjuvant therapy has favorable clinical efficacy in the treatment of DF,which can effectively promote ulcer wound healing,reduce inflammatory response and improve coagulation.关键词
