Abstract
Objective:To study the effect of totally laparoscopic distal gastrectomy(TLDG)and laparoscopic assisted distal gastrectomy(TLDG)in elderly patients with gastric cancer.Method:A total of 60 patients with gastric cancer admitted to the Affiliated Hospital of Jiangnan University from January 2022 to January 2024 were selected for this study.They were randomly divided into full cavity mirror group(n=30)and auxiliary group(n=30)by computer numbering method.TLDG was used in the endoscopic group and LADG was used in the auxiliary group.The results of the two groups were compared(evaluation indicators included surgical indicators,inflammatory factors,immune indicators,complications).Result:The intraoperative blood loss in the total endoscopy group was less than that in the auxiliary group,and the postoperative exhaust time,feeding time and hospitalization days were shorter than those in the auxiliary group,with statistical significance(P<0.05).The levels of serum inflammatory factors in 2 groups were higher than before 3 days after surgery,but the total endoscopy group was lower than the auxiliary group,and the difference was statistically significant(P<0.05).The levels of all immune function indexes in the two groups were lower than those before surgery,but the total endoscopy group was higher than the auxiliary group,with statistical significance(P<0.05).Conclusion:Compared with LADG,the application of TLDG in elderly patients with gastric cancer has a better effect,which can promote the early recovery of patients,reduce the degree of inflammation and immune function decline caused by surgery,and will not increase the risk of complications.关键词
