老年2型糖尿病延续性护理服务设计研究
Study on the Design of Continuity of Care Service for Elderly with Type 2 Diabetes Mellitus

Abstract
This paper combines service design with pain points in continuity of care for elderly type 2 diabetic patients to explore service design interventions in chronic disease control.We conducted follow-up visits to elderly type 2 diabetic patients and their families,and conducted field research in the endocrine department of the hospital to analyze the pain points and needs in the continuity of care for elderly type 2 diabetic patients,and built a continuity of care service system for elderly type 2 diabetic patients.The establishment of the"hospital-family-community"extended care service system for elderly patients with type 2 diabetes mellitus can effectively improve the control of patients'treatment,assist in solving common medical problems in the care,and realize precise glycemic control in the follow-up glycemic control of elderly patients with type 2 diabetes mellitus.关键词
