精准延续护理对糖尿病患者自我管理能力的影响
Effect of accurate continuity of care on self-management ability of dia-betic patients

Abstract
Objective To explore the application effect of accurate continuity of care in patients with diabetes.Methods A total of 100 diabetic patients admitted to the Affiliated Rehabilitation Hospital of Nanchang University from January 2022 to June 2023 were selected as the study objects,and they were divided into control group(50 cases)and observation group(50 cases)according to random number table method.The control group received routine nursing,and the observation group received accurate continuity of care.Both groups received continuous nursing observation for 3 months.Blood sugar levels,self-management ability and quality of life were compared between the two groups.Results Before nursing,there were no significant difference in blood glucose level,self-management ability and quality of life between the two groups(P＞0.05).The blood glucose level of the observation group after nursing was lower than that of the control group,and the difference was statistically significant(P＜0.05).After nursing,the scores of adult health self-management rating scale in the observation group were higher than those in the control group,the difference was statistically significant(P＜0.05).After nursing,the scores of the MOS 36-item short form health survey(SF-36)in the observation group was higher than those in the control group,and the difference was statistically significant(P＜0.05).Conclusion Accurate continuity of care can ef-fectively stabilize the blood sugar level of diabetic patients,further improve the self-management ability of patients,and improve the quality of life.关键词
