术前纤维蛋白原、癌胚抗原、中性粒细胞与淋巴细胞比值联合检测在乳腺癌诊断中的临床价值
Clinical value of preoperative combined detection of fibrinogen,carcinoembryonic antigen,and neutrophil-to-lymphocyte ratio in the diagnosis of breast cancer

Abstract
Objective To explore the application value of preoperative neutrophil-to-lymphocyte ratio(NLR),fibrinogen(FIB),carcinoembryonic antigen(CEA)and multi-parameter diagnostic model in the diagnosis of breast cancer.Methods The data of patients with breast diseases who visited The Second Hospital of Lanzhou University for the first time from October 2022 to October 2024 were retrospectively collected.According to the pathological results,the included subjects were divided into the breast cancer group(198 cases)and the benign breast disease group(200 cases).The joint diagnostic model was constructed by univariate and multi-variate regression to analyze the clinical significance of clinical indicators in the diagnosis of benign and ma-lignant breast lesions.Results Univariate analysis showed that the differences in NLR,FIB,and CEA be-tween the breast cancer group and the benign breast lesion group were statistically significant(P<0.05).Multi-variate analysis found that CEA and FIB were independent risk factors for breast malignancy.The areas under the curve of CEA,FIB,and NLR in the receiver operating characteristic curve were 0.753,0.716,and 0.656,respectively.The three indicators have certain value in the diagnosis of benign and malignant lesions of breast.The area under the curve of the NLR+FIB+CEA combined diagnosis model constructed by Logistic regres-sion increased to 0.842,with a sensitivity and specificity of 78.4%and 78.2%,respectively.In addition,correlation analysis showed that CEA levels were correlated with patient age and progesterone receptor(P<0.05);FIB levels were correlated with age and histological grade(P<0.05);and the combined diagnosis model was correlated with age(P<0.05).Conclusion The combined diagnostic model based on NLR,FIB,CEA and their combination shows high sensitivity and specificity in the differential diagnosis of breast cancer,and the diagnostic efficacy of the combined model is better than that of a single indicator.It can be used as an effec-tive tool for non-invasive preoperative evaluation of breast cancer and has important clinical value.关键词
