开腹与腹腔镜手术在初治卵巢癌患者中的临床疗效分析:一项回顾性研究
Clinical Outcomes of Open versus Laparoscopic Surgery for Primary Treatment of Ovarian Cancer:A Retrospective Study

Abstract
Objective:To compare the clinical outcomes between laparoscopic and open surgery for primary ovarian canc-er treatment,and to identify potential influencing factors.Methods:We conducted a retrospective analysis of clinical data from eligible patients who underwent primary ovarian cancer surgery at Sichuan Cancer Hospital between January 2016 and November 2022.The data of patients undergoing laparoscopic and open surgery were matched at a 1∶2 ratio using propensity score matching.Progression-free survival(PFS)and overall survival(OS)were compared between groups using the Kaplan-Meier method,and prognostic factors were ana-lyzed by Cox proportional hazards regression.Results:After propensity score matching,the final analysis included 195 pa-tients,with 99 in the open surgery group and 50 in the laparoscopic group.The open surgery group demonstrated significantly longer median PFS(48.8 months vs 30.5 months;P<0.001)and OS(72.2 months vs 52.6 months;P<0.001)compared to the laparoscopic group.Univariate analysis identified laparoscopic surgery,serous carcinoma,and advanced-stage disease as significant adverse prognostic factors for PFS,while laparoscopic approach,BMI>24 kg/m2,and postmenopausal status were independently associated with worse OS.Multivariate analysis demonstrated that laparoscopic surgery and advanced tumor stage remained independent predictors of PFS,while the laparoscopic approach was independently associated with OS.Conclusion:Compared with open surgery,the laparoscopic approach was significantly associated with worse oncological out-comes in ovarian cancer patients,demonstrating both inferior PFS and OS,particularly in advanced-stage disease.These findings suggest that laparoscopic surgery should be cautiously considered for ovarian cancer management.关键词
