子宫颈小细胞癌临床病理分析及预后预测模型的构建
Clinicopathological Characterization and Prognostic Modeling in Small Cell Carcinoma of the Cervix

Abstract
Objective:To evaluate clinicopathological prognostic factors and establish a nomogram prediction model for small cell carcinoma of the cervix(SCCC),facilitating clinical management.Methods:We retrospectively analyzed the clinicopathological data of 104 SCCC patients treated at the Affiliated Cancer Hospital of Guangxi Medical University.Inde-pendent prognostic factors were identified through logistic regression analysis,and a Nomogram was developed to score these parameters.Bioinformatic interrogation of public datasets was conducted to delineate the molecular landscape of SCCC.Re-sults:Among 104 SCCC patients,the 3-year overall survival and progression-free survival rates were 57%and 55%,re-spectively,while the corresponding 5-year rates were 48%and 43%.Multivariate Cox regression analysis identified Interna-tional Federation of Gynecology and Obstetrics(FIGO)stage(I~IIA vs IIB~IV:HR=0.53,95%CI:0.33~0.83),lymph node metastasis(negative vs positive,HR=0.63,95%CI:0.42~0.93),distant metastasis(negative vs positive,HR=0.65,95%CI:0.43~0.98),and CgA positivity(HR=1.81,95%CI:1.20~2.76)as independent prognostic factors for SCCC(all P<0.05).Bioinformatics analysis revealed significantly elevated CgA expression in SCCC(expression value:14,739.6,P<0.05,Log2 FC>1),suggesting its association with poor prognosis.The Nomogram produced total scores ranging from 2 to 100 points,with higher total scores indicating greater mortality risk(hazard ratio range:0.50~0.60).Univariate stratified analysis showed that the EP regimen(cisplatin+etoposide),TP regimen(paclitaxel+cisplatin),combined EP+TP chemotherapy,and concurrent chemoradiotherapy were associated with improved survival;however,multi-variate analysis revealed no statistically significant differences in prognosis between patients receiving these treatments and those who did not receive any chemotherapy(P>0.05).Conclusion:FIGO stage,lymph node metastasis,and distant me-tastasis are critical prognostic factors for SCCC,while CgA positivity may indicate adverse outcomes.Radical surgery com-bined with platinum-based chemotherapy remains the core therapeutic strategy.The Nomogram model serves as a visual tool for risk stratification and individualized treatment planning.This study explores potential directions for prognostic stratifica-tion and targeted therapy in SCCC,though prospective studies are needed to validate standardized protocols.关键词
