Abstract
Objective To explore the clinical application of blood-letting puncture in the treatment of acute gouty arthritis(AGA).Methods CNKI,Sinomed,Wanfang database and VIP journal database were searched for the literature on the treatment of AGA by blood-letting puncture from the establishment of the database to November 2023.The database of pricking blood prescription was established.Excel 2019 was used to analyze the frequency and frequency of pricking blood parts and pricking blood operation process.SPSS Modeler 18.0 and SPSS 26.0 were used for correlation analysis and cluster analysis.Cytoscape and dychats.com were used to realize data visualization upgrade.Results A total of 269 literatures were included,and 281 prescriptions were extracted,involving 78 parts.The top three use frequencies of blood-letting puncture parts were Ashi point,superficial stasis at pain point and Taichong.The commonly used specific acupoints were Shu point,Yuan point and Xing point.The meridian tropism was mainly based on the spleen channel of foot-taiyin,the liver meridian of foot-jueyin and the stomach meridian of foot-yangming.The main part was in the lower limbs.Through correlation analysis,9 key acupoints(Ashi point,Taichong,Xingjian,Xiangu,Neiting,Sanyinjiao,Yinlingquan,Xuehai and Taibai)and core compatibility relationship(Taichong-Xingjian-Xiangu)were obtained.Six effective clusters were obtained by cluster analysis.The commonly used blood-letting puncture was the three-edged needle pricking method.The amount of bleeding was usually 1-5 ml.The duration of use with cupping therapy was mostly 5-10 min.In a course of treatment,the general pricking blood 3-5 times,the interval time was about 48 h.Conclusion The idea of blood-letting puncture to treat AGA is mainly based on clearing heat and relieving pain,taking into account clearing liver,regulating spleen and stomach,relieving turbidity and removing blood stasis.Triangular needle pricking method is commonly used,and the intensity of bloodletting is aimed at eliminating its evil.关键词
