新型炎症因子水平与急性脑梗死患者静脉溶栓后预后的相关性研究
Study on the Correlation Between Levels of Novel Inflammatory Factors and Prognosis After Intravenous Thrombolysis in Acute Cerebral Infarction Patients

Abstract
Objective To investigate the relationship between prognosis and levels of novel inflammatory factors in patients with acute cerebral infarction after intravenous thrombolysis.Methods A total of 350 patients with acute cerebral infarction who received intravenous thrombolysis at The First Affiliated Hospital of Henan University of Science and Technology from January 2023 to September 2024 were consecutively enrolled.Based on the modified Rankin Scale(mRS)scores 90 days after thrombolytic therapy,patients were divided into a good prognosis group(mRS≤2,212 cases)and a poor prognosis group(mRS>2,138 cases).Observational indicators included the National Institutes of Health Stroke Scale(NIHSS)score,Systemic Immune-Inflammation Index(SII),Neutrophil to Lymphocyte Ratio(NLR),Platelet to Lymphocyte Ratio(PLR),Neutrophil Percentage to Albumin Ratio(NPAR)levels of inflammatory factors.Multiple logistic regression analysis was used to assess the correlation between poor prognosis after intravenous thrombolysis and inflammatory factor levels.Based on the results of the multivariate logistic regression analysis,a combined indicator was constructed,and Receiver Operating Characteristic(ROC)curve analysis was performed to evaluate the predictive efficacy of different inflammatory markers and other indicators for poor prognosis in patients with cerebral infarction after intravenous thrombolysis.Results A total of 138 patients had a poor prognosis after intravenous thrombolysis,with an incidence of 39.43%.The results of multivariate logistic regression analysis indicated that NIHSS score,SII,NLR,PLR,and NPAR were significant factors associated with poor prognosis(P<0.05).ROC curve analysis revealed that the combined indicator had the highest area under the curve(AUC)at 0.895(0.863-0.927),with a sensitivity of 0.898 and specificity of 0.750.Other novel inflammatory factors such as NPAR,SII,NLR,and PLR also showed certain predictive value.Conclusion Novel inflammatory factors such as NPAR can serve as independent predictors of poor prognosis after intravenous thrombolysis in patients with acute cerebral infarction,and combined detection offers higher predictive value.关键词
