Abstract
Objective To explore the diagnostic value of two-dimensional ultrasound combined with four-dimensional ultrasound in identifying fetal malformations.Methods A retrospective analysis was conducted on 100 pregnant women admitted to Luoyang Maternal and Child Health Hospital from June 2023 to August 2024,all of whom underwent both two-dimensional and four-dimensional ultrasound examinations.The detection rates of malformed fetuses were recorded,and the diagnostic results of two-dimensional ultrasound alone and in combination with four-dimensional ultrasound for fetal malformations were compared,using the outcomes of induction or delivery as the gold standard to analyze the diagnostic efficacy of the combined ultrasound methods.Results The outcomes of induction or delivery revealed a total of 21 cases of fetal malformations(21.00%).Among these,4 cases involved facial malformations(4.00%),4 cases involved central nervous system malformations(4.00%),4 cases involved circulatory system malformations(4.00%),and 4 cases involved skeletal malformations of the limbs(4.00%),with 5 cases classified as other malformations(5.00%).The Kappa value of two-dimensional ultrasound,four-dimensional ultrasound and two-dimensional ultrasound combined with four-dimensional ultrasound in detecting all kinds of fetal malformations is 0.564,which indicates that the consistency is acceptable.The sensitivity of two-dimensional ultrasound in diagnosing fetal malformations was 47.62%,the specificity was 97.47%,and the accuracy was 87.00%;for four-dimensional ultrasound,these values were 76.19%sensitivity,98.73%specificity,and 94.00%accuracy;while the combined examination yielded a sensitivity of 90.48%,specificity of 98.73%,and an accuracy of 97.00%.The sensitivity and accuracy of the combined examination for diagnosing fetal malformations were significantly higher than those of two-dimensional ultrasound alone(P<0.05).Conclusion The combination of two-dimensional and four-dimensional ultrasound demonstrates high sensitivity and accuracy in diagnosing various types of fetal malformations,providing valuable reference for clinical screening.关键词
