Abstract

Objective To investigate the impact of preoperative fusion on long-term postoperative exodrift in children with basic type of intermittent exotropia(IXT).Methods Retrospective cohort study.The subjects were all children with IXT who underwent the recession-plication technique at the Department of Ophthalmology,Beijing Children's Hospital,Capital Medical University,between January 2022 and December 2023.Clinical data were collected from patients who com-pleted a follow-up exceeding more than 1 year.The participants were divided into two groups by the presence or absence of preoperative fusion:the fusion compensation group and the fusion decompensation group.Comparisons were made regarding postoperative exodrift,success rates,and stereoacuity between the two groups.Results The study included 93 patients,with 71 patients in the fusion decompensation group and 22 patients in the fusion compensation group.The deviation for the fusion decompensation group was-36.65±10.35PD for distance,-44.79±10.87PD for near,respectively.In contrast,the deviation for the fusion compensation group was-29.55±10.23PD for distance and-39.32±10.27PD for near,respectively.The fusion decompensation group demonstrated significantly greater preoperative deviation compared to the fusion compensation group(Pdistance=0.012,Pnear=0.001).Long-term follow-up data indicated that the fusion decompensation group experienced greater exodrift(10.41±9.31 PD for distance,10.73±9.72 PD for near)compared to the fusion compensation group(6.68±6.21 PD for distance,6.18±7.21PD for near)(Pdistance=0.040,Pnear=0.028).The long-term success rate postoperative was 66.20%for the fusion decompensation group and 77.27%for the fusion compensation group,with no statistically significant difference observed between the two groups(P=0.087).Notably,patients with preoperative fusion decompensation exhibited a significant improvement in stereoacuity following surgery(P<0.001),with no significant difference in the stereoacuity between the two groups at the final follow-up(P=0.469).Conclusions For children with IXT,the larger preoperative deviation is associated with a higher likelihood of fusion decompensation and increased postoperative exodrift.Nevertheless,patients with preoperative fusion decompensation still demonstrate significant improvements in postoperative stereoacuity.
