Abstract

Objective To explore the changes of hypothalamic-pituitary-thyroid(HPT)axis hormones and hypothalamic-pituitary-gonad(HPG)axis hormones in patients with depressive comorbidities obstructive sleep apnea(OSA).Methods A total of 182 patients with first-episode depression admitted to the clinical psychology Department of the People's Hospital of Xinjiang Uygur Autonomous Region from September 2022 to September 2024 were screened and finally included by the Yidu Cloud system.According to the ventilation index(AHI)≥5,the patients were divided into depression group(92 cases)and comorbid group(90 cases).A total of 78 OSA patients without depression admitted during the same period were selected as the OSA group,and 48 healthy subjects without OSA,depression or other underlying diseases were selected as the healthy control group).General information about all subjects was collected and recorded,including gender,age,BMI,and whether or not they smoked.5 ml of fasting venous blood of the subjects was collected and thyroid hormones,including total thyroxine(T4),triiodothyronine(T3),free triiodothyronine(FT3),free thyroxine(FT4)and thyroid stimulating hormone(TSH),were detected by German Roche e801 automatic electrochemiluminescence analyzer.At the same time,the American Beckman DXO800 automatic chemiluminescence analyzer was used to detect gonadal hormones,including estradiol(E2),progesterone(P),luteinizing hormone(LH),follicle stimulating hormone(FSH),prolactin(PRL)and testosterone(T).Results There were significant differences in T3 levels among the 4 groups(P<0.05).Further pair comparison showed that T3 levels in comorbidities group,OSA group and depression group were lower than those in healthy group,and T3 levels in comorbidities group were lower than those in depression group and OSA group,with statistical significance(P<0.05).There were significant differences in PRL levels among the 4 groups(P<0.05).Further pairwise comparison showed that PRL levels in comorbidities group,OSA group and depression group was higher than that in healthy group(P>0.05).Multivariate Logistic regression analysis showed that T3 was a protective factor for OSA comorbidity depression,and the probability of comorbidity increased by 0.212 times for each unit reduction of T3,95%CI(0.070~0.644),and the above indicators were statistically significant(P<0.05).Conclusion Comorbidities of OSA affect hormone levels in patients with depression,mainly reflected in decreased T3 levels.
