摘要

Abstract

In current medical practice,with the extensive application of laparoscopic technology in elderly patients,postoperative cognitive dysfunction(POCD)has become a key problem affecting the rehabilitation and quality of life of elderly patients.Permissible hypercapnia(PHC)is an acceptable respiratory acidosis resulting from the use of low tidal volume protective ventilation strategies.Clinical studies have shown that the application of PHC in laparoscopic surgery can improve postoperative cognitive function in elderly patients through various mechanisms.It is important to note that although PHC has shown potential benefits in multiple studies,several key issues regarding its use in elderly patients remain unclear:first,the mechanism of overall effect of PHC on the physiological status of elderly patients has not been fully elucidated;secondly,the specific role of PHC strategy in postoperative pain management and its potential impact on postoperative cognitive function still need to be further verified;finally,there is still a lack of clear clinical consensus on key parameters such as allowable limits and safe limits for CO2 during PHC therapy.In this paper,we reviewed the influence of PHC ventilation strategy on elderly patients undergoing laparoscopic surgery and the related mechanisms.
