HD-tDCS联合反复促进疗法在脑卒中恢复期患者中的应用效果
Application Effect of HD-tDCS Combined with Repetitive Facilitative Exercise on Stroke Patients in Recovery Period

Abstract
Objective:To investigate the application effect of high definition transcranial direct current stimulation(HD-tDCS)combined with repetitive facilitative exercise(RFE)on stroke patients in recovery period.Method:A total of 123 stroke hemiplegia patients in recovery period treated in the First Hospital of Shanxi Medical University from May 2022 to May 2024 were selected and divided into conventional group,HD-tDCS group and combined intervention group by random number table method,with 41 cases in each group.The conventional group received routine rehabilitation training,the HD-tDCS group received routine rehabilitation training and HD-tDCS intervention,and the combined intervention group received HD-tDCS combined RFE intervention on the basis of the conventional group.The limb hemiplegia degree(Brunnstrom stage),hand function[action research arm test(ARAT)],upper limb motor function[Fugl-Meyer assessment upper limb(FMA-UL)],surface electromyography indexes[root mean square(RMS)and integrated electromyography(iEMG)of surface electromyography of wrist extensor and extensor digitorum]and daily living ability[modified Barthel index(MBI)]before and after intervention were compared among the three groups.Result:After intervention,there was significant difference in Brunnstrom stage among the three groups(P<0.05).After intervention,the proportion of stage Ⅵ in the combined intervention group was higher than that in the conventional group(58.54％vs 21.95％),the difference was statistically significant(P<0.05).After intervention,ARAT scores,FMA-UL scores,RMS,iEMG levels of wrist extensor and extensor digitorum were all increased in three groups,and those in the combined intervention group were higher than those in the HD-tDCS group and conventional group,MBI scores were increased in three groups,and that in the combined intervention group was higher than that in the conventional group,the differences were statistically significant(P<0.05).Conclusion:HD-tDCS combined with RFE can improve the degree of stroke hemiplegia patients in recovery period,promote the recovery of hand function and upper limb motor function,enhance the degree of neuromuscular excitation of upper limb,and improve the self-care ability of daily life.关键词
