Clinical Efficacy of Early Immune Enteral Nutrition Combined with Short-term Pulmonary Rehabilitation in the Treatment of AECOPD Patients with Mechanically Ventilated

Abstract
Objective:To observe the clinical efficacy of early immune enteral nutrition combined with short-term pulmonary rehabilitation in patients undergoing mechanical ventilation for acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Method:Ninety AECOPD patients who met the inclusion criteria in the Intensive Care Unit of Jiangxi Provincial Hospital of Traditional Chinese and Western Medicine from March 2,2023 to June 2,2024 were selected,and they were randomly assigned to three groups,with 30 patients in each group.The conventional group(group A)received standard enteral nutrition,the control group(group B)received immune enteral nutrition,and the experimental group(group C)received additional short-term pulmonary rehabilitation on the basis of group B.At day 1 of enrollment(T1),day 5 of enrollment(T2),day 7 of enrollment(T3),and day 10 of enrollment(T4),the acute physiology and chronic health evaluation Ⅱ(APACHEⅡ)score,clinical pulmonary infection score(CPIS),serum albumin(ALB),prealbumin(PA),procalcitonin(PCT),C reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),serum T lymphocyte subsets CD3+,CD4+,CD8+,CD4+/CD8+levels and diaphragm function were observed.Result:After treatment,APACHEⅡ score,CPIS,CRP,PCT,IL-6,TNF-α,CD3+,CD4+,CD8+and CD4+/CD8+,diaphragmatic mobility during quiet breathing,diaphragmatic thickness at the end of breathing and diaphragmatic thickening rate were significantly improved among the three groups(Pintergroup<0.05,Pinteraction<0.05),with group C demonstrating superior outcomes compared to group A and group B at T4(P<0.05).After treatment,there were no significant differences in the incidence of ventilator-associated pneumonia(VAP),alanine transaminase(ALT),and serum creatinine(Scr)levels among the three groups(P>0.05),while group C exhibited shorter mechanical ventilation duration and ICU length of stay compared to group A and group B(P<0.05).Conclusion:Early implementation of immune enteral nutrition combined with short-term pulmonary rehabilitation has good clinical effect in AECOPD patients with mechanically ventilated.关键词
