Abstract
Objective:To explore the application value of CT perfusion imaging in the evaluation of cerebral blood flow in patients with carotid stenosis(CS).Method:A retrospective study was conducted on the clinical data of 100 patients with CS admitted to Renhuai People's Hospital from March 2020 to May 2024,the patients were divided into healthy side group(n=30,input from the M1 segment of healthy middle cerebral artery),affected side group(n=30,input from the M1 segment of middle cerebral artery on the narrow side)and the basal group(n=40,input from the basal artery)according to different CT perfusion input arteries.All patients underwent cerebral CT perfusion scan,and different input arteries were selected for image post-processing.The mean flow velocity(MFV),pulsatility index(PI)of middle cerebral artery and collateral circulation opening of patients with different degrees of CS were compared.The cerebral blood volume(CBV),time to peak(TTP),cerebral blood flow(CBF),mean transit time(MTT)and delay time(DT)of healthy side and affected side were compared in the three groups of patients with mild and moderate stenosis,severe stenosis and complete occlusion.Result:There were significant differences in MFV,PI and collateral circulation opening rate among the four groups(P<0.05).MFV and PI in mild stenosis group were smaller than those in moderate stenosis group,those in moderate stenosis group were smaller than those in severe stenosis group,and those in severe stenosis group were smaller than those in complete occlusion group(P<0.05).The opening rate of collateral circulation in the complete occlusion group was higher than that in the mild stenosis group,moderate stenosis group and the severe stenosis group,and that in the severe stenosis group was higher than that in the mild stenosis group and the moderate stenosis group,the differences were statistically significant(P<0.05).In patients with mild and moderate stenosis,there were no significant differences in CBF,CBV,MTT,TTP and DT between the healthy side and the affected side(P>0.05).In patients with severe stenosis and complete occlusion,CBF and CBV in the basal group were significantly larger than those in the healthy side group and the affected side group,MTT,DT and TTP were significantly shorter than those in the healthy side group and the affected side group,the differences were statistically significant(P<0.05).Conclusion:For patients with mild or moderate CS,any large artery can be selected as the input artery for CT perfusion imaging,but for patients with severe or completely occlusive CS,the basilar artery is preferred as the input artery.关键词
