Effect of Bone Cement Filling Combined with Screw Fixation on Bone Metabolism in Patients with Medial Tibial Plateau Bone Defects in Knee Arthroplasty

Abstract
Objective:To investigate the effect of bone cement filling combined with screw fixation on bone metabolism in patients with medial tibial plateau bone defects during knee arthroplasty.Method:A retrospective analysis was conducted on the data of 85 patients with medial tibial plateau bone defects who underwent knee arthroplasty at People's Hospital of Ganzhou District from January 2016 to January 2022.According to different intraoperative treatment methods,43 patients who received bone cement filling combined with screw fixation were divided into an observation group,and 42 patients who received autogenous bone transplantation were divided into a control group.The perioperative indicators,knee joint function scores,joint range of motion,bone metabolism indicators,and incidence of complications between the two groups were compared.Result:The operation time and average healing time of the observation group were shorter than those of the control group,and the weight-bearing time was earlier than that of the control group,the differences were statistically significant(P<0.05).There was no statistically significant difference in intraoperative bleeding between the two groups(P>0.05).The complication rate of the observation group was lower than that of the control group,the difference was statistically significant(P<0.05).At 6 months after operation,the knee joint function scores(clinical score,functional score)and joint range of motion increased;bone alkaline phosphatase/alkaline phosphatase(BALP/ALP)increased and β-collagen degradation product(β-CTX)decreased 14 days after operation in both groups;those in the observation group were better than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Bone cement filling combined with screw fixation is used to treat the medial tibial plateau bone defects in knee arthroplasty,which can regulate bone metabolism,accelerate bone healing,improve knee joint function,and reduce complications.关键词
