Rapid Health Technology Assessment of Finerenone in the Treatment of Type 2 Diabetes-Related Chronic Kidney Disease

Abstract
Objective To rapidly evaluate the efficacy,safety and economy of finerenone in the treatment of type 2 diabetes mellitus(T2DM)-related chronic kidney disease(CKD).Methods Embase,PubMed,The Cochrane Library,INAHTA,CRD,CNKI,VIP and Wanfang databases were searched,with a search period from the establishment of each database to November 2023.AMSTAR scale,CHEERS scale and HTA checklist were used to evaluate the quality of systematic review(SR)/Meta-analysis,economic evaluation and HTA report of finerenone in the treatment of T2DM-related chronic kidney disease.Results A total of 14 articles were included,including 12 systematic reviews/Meta-analyses,1 HTA report and 1 pharmacoeconomic study.In terms of effectiveness,compared with placebo,finerenone could significantly reduce renal comprehensive indicators and the risk of cardiovascular events(P<0.05).Compared with angiotensin-converting enzyme inhibitor(ACEI)/angiotensin Ⅱ receptor blocker(ARB)monotherapy,finerenone combined with ACEI/ARB therapy could significantly reduce urinary albumin/creatinine ratio(P<0.05).In terms of safety,there was no significant difference in the incidence of adverse events between finerenone and placebo group(P>0.05).Although the incidence of hyperkalemia was slightly higher than that of the placebo group,it was still significantly lower than that of other mineralocorticoid receptor antagonists(P<0.05).In terms of economy,finerenone combined with basic treatment had economic advantages compared with basic treatment alone.Conclusion finerenone has certain efficacy and safety in the treatment of T2DM-related chronic kidney disease.The economy needs to be determined according to the health resources of different countries or regions.It is necessary to further carry out relevant economic research in China.关键词
