基于"颈骶并治"超声引导下针刀治疗神经根型颈椎病的临床研究
Clinical Study of Ultrasound-Guided Acupotomy in Treatment of CSR Based on'Concurrent Treatment of Cervical and Sacral Regions'

Abstract
Objective:To observe the clinical effect of ultrasound-guided acupotomy in treatment of cervical spondylotic radiculopathy(CSR)based on'concurrent treatment of cervical and sacral regions'.Methods:87 patients with CSR were randomly divided into the control group(n=43,two cases dropped off)and the observation group(n=43,on case dropped off).The control group was treated with ultrasound-guided T-shape acupotomy,and the observation group was treated with ultrasound-guided acupotomy based on concurrent treatment of cervical and sacral regions.Both groups were treated once every 5 days for a total of 3 times.VAS score,CSR20 score,cervical curvature D-value,and serum level of IL-6 were compared between the two groups before and after the treatment,and the clinical efficacy and safety were evaluated.Results:Compared with those before the treatment,the CSR20 score and D value were increased(P＜0.01),the VAS score was decreased(P＜0.01),and the serum level of IL-6 was decreased after the treatment in the two groups(P＜0.05).The CSR20 score was higher(P＜0.01),the VAS score was lower(P＜0.05),and the D value was higher in the observation group than that in the control group(P＜0.05);there was no statistical difference in the serum level of IL-6 between the two groups(P＞0.05).The D value difference between before and after the treatment was higher in the observation group than that in the control group(P＜0.01).There was no statistical difference in the serum level difference of IL-6 between of the two groups(P＞0.05).In terms of the total effective rate,it was 93.2％(41/44)in the observation group and 88.4％(38/43)in the control group,in which there was no statistical significance between the two groups(P＞0.05).There was no adverse reaction during the treatment process in both groups.Conclusion:The efficacy of the'concurrent treatment of cervical and sacral regions'acupotomy and the'T-shape'acupotomy exhibit comparable efficacy in treatment of CSR,and their effects in decreasing serum level of IL-6 is comparable,but the former therapy has a significant advantage in relieving the degree of cervical pain,and improving the symptoms,signs and cervical physiological curvature,and there are no adverse effects.关键词
