Abstract
Objective:To explore the clinical efficacy of nasal high flow heating humidification and oxygen inhalation and routine treatment in patients with chronic obstructive pulmonary disease(COPD)and mild type Ⅱ respiratory failure(RF).Method:Sixty-four patients with COPD combined with mild type Ⅱ RF treated in Beijing Huairou Hospital from October 2022 to October 2024 were selected and divided into study group(n=32)and control group(n=32)by random number table method.The control group received conventional treatment,and the study group received nasal high flow heating humidification and oxygen inhalation combined with routine treatment.Clinical indexes,blood gas indexes,lung function,6-minute walking test(6MWT),respiratory rate(RR),disease severity and adverse reactions were compared between the two groups.Result:The hospital stay time,dyspnea improvement time and oxygen inhalation time of the study group were shorter than those of the control group,and the differences were statistically significant(P<0.05).After treatment,arterial partial pressure of oxygen(PaO2),arterial oxygen saturation(SaO2),pH value and oxygenation index(PaO2/FiO2)of the two groups increased,while arterial partial pressure of carbon dioxide(PaCO2)decreased,PaO2,SaO2,pH value and PaO2/FiO2 of the study group were higher than those of the control group,and PaCO2 was lower,the differences were statistically significant(P<0.05).After treatment,the forced exhaled air volume in the first second(FEV1),forced vital capacity(FVC),total lung volume(TLC)and maximum peak respiratory velocity(PEFR)of the two groups were increased,and the study group was higher than the control group,the differences were statistically significant(P<0.05).After treatment,the 6MWT distance of the two groups increased,and the RR decreased,the 6MWT distance of the study group was further than that of the control group,and the RR was lower,the differences were statistically significant(P<0.05).After treatment,the modified dyspnea index(mMRC)and COPD evaluation test(CAT)scores of the two groups decreased,and the study group was lower than the control group,the differences were statistically significant(P<0.05).The incidence of adverse reactions in the study group was 9.38%,which was lower than 31.25%in the control group,and the difference was statistically significant(P<0.05).Conclusion:The combination of nasal high flow heating humidification and oxygen inhalation and routine treatment can promote symptom relief,improve lung function,respiratory rate,blood gas indicators,and reduce the incidence of adverse reactions in COPD patients with mild type Ⅱ RF.关键词
