消化道恶性肿瘤PICC化疗患者非计划拔管的影响因素
Factors influencing unplanned extubation in patients undergoing PICC chemo-therapy for gastrointestinal malignancies

Abstract
Objective To investigate the factors influencing unplanned extubation(UE)in patients undergoing transperipheral venous peripherally inserted central catheter(PICC)for chemotherapy of gastrointestinal malig-nancies.Methods A total of 513 patients with PICC chemotherapy admitted to our hospital from February 2019 to January 2024 were included.Their clinical data were collected,the influencing factors and causes of UE were analyzed by the Logistic regression model,and the receiver working characteristics(ROC)curve was used to an-alyze the accuracy of predictive variables.Results Of the 513 patients,467 were normally extubated and 46 were UE,with a UE rate of 8.97%(46/513).The multivariate results of Logistic regression model showed that age,D-dimer(D-D)level,external indwelling catheter length 2 cm U fixation,catheter prolapse,persistent postop-erative fever,catheter-related,catheter-related infection,and catheter-related thrombosis were independent risk factors for UE in PICC chemotherapy(P＜0.05).ROC results showed that the above independent risk factors and the combined prediction of PICC chemotherapy patients were statistically significant(P＜0.05),with the combined prediction AUC=0.972,95%CI of 0.941-1.000,sensitivity of 0.935 and specificity of 0.940.Conclusion The independent risk factors for UE in patients with PICC chemotherapy are age,D-D level,in-dwelling catheter length 2 cm U,catheter prolapse,and persistent postoperative fever,etc.We should pay great attention to it and strengthen monitoring and management to reduce the risk of UE and ensure the safety of treat-ment.关键词
