温针灸联合舒络通脉汤治疗寒凝血瘀型糖尿病周围神经病变疗效分析
Therapeutic Efficacy of Warm Acupuncture Combined with Shuluo Tongmai Decoction in Treating Diabetic Peripheral Neuropathy of Cold Congealing and Blood Stasis Type:A Clinical Analysis

Abstract
Objective:To observe the clinical efficacy of warm acupuncture combined with Shuluo Tongmai Decoction in treating diabetic peripheral neuropathy(DPN)of cold coagulation and blood stasis type,and its impact on nerve conduction velocity,homocysteine(Hcy),and cystatin C(CysC).Methods:Eighty patients with cold coagulation and blood stasis type DPN who met the inclusion criteria were selected and randomly assigned into treatment and control groups,with 40 cases in each group.The control group received injections of alpha-lipoic acid,while the treatment group received warm acupuncture combined with foot baths of Shuluo Tongmai Decoction in addition to alpha-lipoic acid.Both groups were treated for 14 days.Changes in traditional Chinese medicine symptom scores,nerve conduction velocity,Hcy,CysC levels,and Toronto Clinical Scoring System(TCSS)scores before and after treatment were compared between the two groups,with safety monitoring conducted.Results:Both groups showed significant decreases in traditional Chinese medicine symptom scores after treatment(P<0.05),with greater improvement in the treatment group compared to the control group(P<0.05).Nerve conduction velocities in both groups significantly increased after treatment(P<0.05),with the treatment group showing greater improvement compared to the control group(P<0.05).Levels of Hcy and CysC significantly decreased after treatment in both groups(P<0.05),with no statistically significant difference between the groups(P>0.05).TCSS scores decreased significantly in both groups after treatment(P<0.05),with the treatment group showing lower scores compared to the control group(P<0.05).The total effective rate was 92.5％(37/40)in the treatment group and 67.5％(27/40)in the control group,with a significant difference favoring the treatment group(P<0.05).No adverse reactions were observed in either group during the treatment period.Conclusion:Warm acupuncture combined with Shuluo Tongmai Decoction foot baths effectively alleviates clinical symptoms and signs,improves nerve conduction velocity,and demonstrates good safety in treating cold coagulation and blood stasis type DPN.关键词
